I'm 50 and I've never seen a mod drop as you say. Mod schematics drop, but not actual mods. Any mods you obtain without buying or crafting come from quest rewards.

You're right, you dont know anything about TOR crafting. There IS failure in TOR crafting but it does not come from crafting the items. It comes from obtaining the schematics FOR the good items.
