Would Mittani's name be painted as crap all over EVE forums for the next two weeks if he did this as Mittani, in the game, with no ties to the CSM at all? Eh, maybe. He's important enough that word would spread and enough people would still be offended by what he did that it would garner a little attention... but at the end of the day, as you said, so many people do this that you'd drive yourself insane if you got upset about everyone who did. Would it have spread over tons of media outlets, other forums, etc? No way. You just proved that, considering all the things Darius did that even I, as an EVE player of several years who regularly visits EVE forums, had never heard of before. Mittani isn't just another Alliance leader. He was the CSM chairman, a representative for CCP's EVE playerbase and someone who was broadcast internationally in that position as telling a bunch of people that if they wanna get a guy to kill himself, here's his name to do it.
Then he gets confused as to why people are so mad. And it is for that last part that I, personally, am so disgusted by him. It's bad that he did what he did, but its worse that he not only won't completely own up to it but instead is trying to downplay it. If not for that, there would probably be a whole lot less Mittani hate going around.
In any case, he got his punishment. He's banned for 30 days, removed from CSM (even though he can run again next year and there are more than enough goons to get him in again), and flooded twitter with an embarrassingly large amount of tears the first couple of days, which everyone noticed and lulzed at him for, so I'd say CCP did a sufficient job of letting him know whats up. Beyond that, all we can do is accept that he'll never understand why what he did was wrong, his followers likely won't either, and just hope CCP watches next time he is on CSM to make sure it doesn't happen again.

The thing is though most people and game companies, while they may give tolerance of one degree or another to the above have the common sense and intelligence to reengage their "filter" once they reenter the real world. This is especially important for companies because many of the things that are acceptable in their virtual world are civilly or even criminally actionable in the real world.
It seems that Mittens lost this filter sometime ago. rather than having his virtual persona be an outlet for his anti-social tendencies, he allowed his virtual persona to become ascended, likely because he got more reward (financial, emotional etc.) in the virtual world than he did in the real world. Lets be honest, people in the virtual world applauded him, even when he was being a completely narcissistic ****** bag.
It's like people I deal with regularly now. They peaked in High School. They were bullies and jerks BUT in the High School culture they were rewarded for it. Unlike some they didn't realize when they left high school this was no longer acceptable. They can't hold down a job and find themselves getting into fights constantly. Since they can't get a steady job and they get into drunken brawls all the time they are on a constant cycle of "arrest>bail out> arrest> bail revocation>sentence>jail time>probation/parole>violate probation/parole>jail time...wash rinse repeat" largely because for 18 years being an idiot was rewarded and they have no clue how to deal with the consequences when this is no longer the case.
