Default RIFT in a couple minutes of reading! Extended Edition!

Here I'll address the stuff that wasn't in the Main Story Line (So yes it is really long.) in an order that sort of makes sense.

    Freemarch
Contains Meridian, a city that was once thriving with Abyssal, but during the invasion of Death, the Defiants fled to Freemarch and drove the Abyssal out and claimed the city for themselves. Here Alsbeth resurrects Jakub The Iron Tyrant, once a ruthless dictator for the people of Freemarch, now commanding legions of Death. Ascended defeat Jakub after a bunch of running around talking to people then celebrate with the people of Freemarch.

    Stonefield
After the fight in Freemarch, the Ascended proceed to Stonefield, a place where Titans once roamed, and where Giants, Trolls, Trogolodytes and Rams still roam. Researchers here attempt to open a gate from the Plane of Earth and summon a Titan, which they then plan to bind to their will, which fails (Not seen that one before.) as the Titan Centius they summon, goes on a rampage, causing the Ascended to have to deal with it.

    Scarlet Gorge
Defiants and Guardians both arrive in Scarlet Gorge and find the people enslaved by the Golden Maw, followers of Laethys who are mining for some stuff (Yes, I said it, STUFF.), following their defeat the Ascended proceed to Rock Ridge where the Wanton Maw have opened a rift and begun invading, Ascended beat them too.

    Moonshade Highlands
Dwarves. Dwarves. Dwarves. Dwarves as far as the eye can see. Oh, and they're biased to Guardians, so rude!

The Dwarves of Moonshade originally prospered within the walls of Hammerknell Fortress, until of course, Akylios and his madmen arrived, following their defeat however, Akylios was bound in the deepest recesses of Hammerknell, while any remaining dwarves were cursed. Oh and some Undead Endless somehow end up filling most of the Fortress. Back in present day, the Ascended find the Dwarves facing off against the legions of Tidelord Brenin as they attempt to take Hammerknell and free their master Akylios. Brenin is killed, and the Abyssal driven away (Till slightly later)

    Scarwood Reach
Life and Death Planes both begin arriving in Scarwood Reach in an attempt to gain some special powerful stuff in King's Breach, the life rift outside is closed, the Aelfwar defeated, while the Death cults remain in King's Breach (Till later.).

    Droughtlands
Maelforge dispatches legions of Dragonians to attack Lantern Hook and the Arcane Hand stationed there, Ascended arrive and defeat them.

    Shimmersand
Golden Maw start raiding all over Shimmersand because it has wealthy merchants and the cults of the greedy Dragon of Earth would obviously want that money they have. Ascended defeat them, the end.

    Iron Pine Peak
Here lies the prison of Crucia, within the center of the icy lake. Her whispers reach out to the residents of Iron Pine Peak's villages and the Ascended have to stop them from doing anything stupid. Oh and some Abyssal are there too, in masses. They die in masses too.

    Stillmoor
Stillmoor is mostly considered to be the Endless' Home Base, primary at the Endless Citadel. Also on the hilltop behind it is the cave where Greenscale is imprisoned. Ascended arrive, start mowing down the hills of undead as they go down, they then hack and slash through towns of more undead then arrive at Zareph's Retreat. Guardians and Defiants by this point have joined up to a decent extent, so they go raid the Citadel, enter River of Souls, defeat Alsbeth and the other Endless leaders then go to the Eye of Regulos and destroy Regulos' body.

    Ember Isle
Hosts the volcano where Laethys and Maelforge are imprisoned, this island was lost by the Kelari quite a while ago when the Wanton and Golden Maw invaded, in an attempt to free their masters. Ascended slay their armies and eventually enter the volcano.

    Greenscale's Blight Raid
Ascended go into Greenscale's Prison and defeat Hylas and the remaining House Aelfwar leaders, House Aelfwar actually falls this time. They then proceed to go kill Greenscale, which they do.

    Hylas
Leader of House Aelfwar, a group of rogue Elves who abandoned Silverwood, and is also the leader of the Life Cults for the Mathosia section of the story.

    Greenscale
Dragon of Life, desires to turn the world into a jungle so he can eat as many luggo's and other desirable food he wants. Oh and he prefers his naps in jungles.

    Drowned Halls Raid
In an alternate reality of Lord's Hall in Scarwood Reach, the Abyssal cultists led by High Priestess Hydriss are plotting to invade Telara in the original reality (Our reality I guess.), Ascended go kill Hydriss.

    Rise of the Phoenix Raid
In another alternate reality set in the Last Valley of Stonefield, some priests go to the gate at which Centius was summoned in the original reality. From the gate arrives General Silgen and Wanton Maw legionaries, who then kill all the priests and wants to invade Telara. Ascended defeat all of these guys including General Silgen, later to find that High Priest Arakhurn has not died, but rather, become a phoenix of the Wanton Maw (Logic?). He gets killed too.

    River of Souls Raid
Ascended enter a realm on the edge of the Plane of Death and face off against the Endless leaders and Alsbeth. Regulos then laughs at them when Alsbeth is dead, making remarks about how useless she was anyways.

    Alsbeth
Queen of the Endless. Betrayed the Mathosians and joined the Endless as their Queen so she could devote herself to Regulos under the belief she could rule the world when Regulos "remakes it".

    Hammerknell Fortress Raid
Abyssals struck a deal with Alsbeth that they get to free Akylios in exchange for her being granted the powerful minions of Death that roam within (This deal is also the reason all the Water mobs are upstairs and the Death mobs downstairs, but the deal in itself makes no sense looking at the exchanges...). Jornaru eventually frees Akylios and both are slain by the Ascended. Oh, and all the Death/Water guys inside and on the roof are dead too.

    Jornaru
Akylios' faithful servant, and possibly the second craziest entity in RIFT (The first being Akylios, who turns people crazy on sight.). Spends centuries manipulating the dwarves and corrupting some of them so they can free Akylios.

    Akylios
Unlike the other Blood Storm Gods who were of Azdah's brood, Akylios was born of an Akvan Matriarch. His insanity and thirst for knowledge is boundless and his true intentions in themselves were never really explained, even to the point of his death.

    Gilded Prophecy Raid
In yet another alternate reality the Ascended find that the Golden Maw are planning an invasion on Telara (Didn't we just go through this already?) and the Ascended defeat them.

    Primeval Feast Raid
In another alternate reality set in the Droughtlands, Fae Lord Twyl and his army of Fae are not trying to invade Telara. (Yet.)

Greenscale is slain by Twyl and House Aelfwar falls under Twyl's betrayal, he and the rest of the Fae then consume Greenscale's flesh (Which is apparently limitless in supply.) and gain a mastery over the Plane of Life. (They might have planned to invade Telara eventually.) Ascended then arrive and save Hylas, who then asks them to kill him so he can become a ghost and help them with his ghostly powers (Oooooh....spoooooky...). The Life Anchors are brought down, preventing the Fae's immortality from being of any use (It was already useless against goblin corpses stuffed with dynamite and knives to the back) and eventually the Ascended confront Twyl, who is now Master of Life. Twyl dies.

    Twyl, Master of Life
Hey look! It's Faelord Twyl from the Realm of the Fae Dungeon! This time he isn't a minor character. Empowered by the flesh of Greenscale, whom he slay himself, he planned to become the new ruler of the Plane of Life, but failed when the Ascended came and killed him.

    Atrophinius
Like various other appearances, Atrophinius' role here isn't really explained, other than somebody promising him endless supplies of mead.

Infernal Dawn Raid (Best Raid ever!!)
Not set in an alternate reality inside Mount Carcera on Ember Isle, the Wanton and Golden Maw are able to successfully free Laethys and Maelforge. Ascended proceed inside, and with the help of Carcera, Spirit of the Mountain, slay the Wanton and Golden Maw Generals, and eventually, Laethys and Maelforge.

    Ember Conclave

    Witchlord Szath
Leader of the Goblins who serve the Plane of Fire.

    Packmaster Nahoth
Leader of the Dragonians who serve the Plane of Fire.

    Emberlord Ereetu
Leader of the original Wanton who arrived from the Plane of Fire, as well as the central figure within the Conclave trio.

    Ituziel
    Guardian of the Ember Conclave.

    Rusilia Dreadblade
Captain of the Dread Fortune, a colossal ship constructed to sail the seas after they are transformed into magma, so it may strike at distant lands unaffected by Maelforge's fury.

    Maklamos the Scryer
A Cyclops who gained the power to see into the future.

    Laethys
God of Earth and boundless greed. Her desire is to attain all wealth she possibly can, as well as infinite beauty amongst herself.

    Maelforge
God of Fire and holds a desire to burn everything on Telara. He is hinted to hold feelings for Laethys, as shown in the artifact "Maelforge's Love Letters to Laethys" and unless those Dragon Eggs were asexually produced, I think there's something going on here.

    Intrepid: Drowned Halls Raid
Set in an alternate reality where Hydriss and her invasion forces have been powered up to Level 62 and still end up dying.

Triumph of the Dragon Queen Raid
In an alternate reality (How many do Trion plan to add!?) of the Tower of Dawn in Kingsward, Crucia has conquered Telara and returned to the Plane of Air, leaving Commisar Typhiria with a promotion to General Typhiria and who we once knew as Jultharin, Caelia's servant back in Charmer's Caldera, has killed Caelia and serves Typhiria in hopes of promotion and more power. Telarans led by Cyril Kalmar and Asha Catari launch an attack on the Tower of Dawn which progressively fails towards the end. Simon, Telara's Ultimate Adventurer, was the first to die (How unpredictable...), but his ghost is able to aid the Ascended towards the General's Throne Room where they kill Jultharin and Typhiria.

    Grim Awakening Raid
Set in an alternate reality of Kingsward......................................... ..........................Really? Another one!?

Salvarola has formed a demonic pact with an entity known as Ultane who then has him destroy Princess Isabella. The Ascended arrive when Prince Casimar is attempting to talk some sense into his brother, Prince Kaliban, who is then trying to talk some sense into him, who then enrages Casimar who then enrages Ultane who then turns Kaliban into Magma Lord Kaliban who then gets angry and breaks Casimar's skull and then gets killed by the Ascended who are hopefully avenging him (Hope they all take anger management classes in the next life.). Ascended then face off against Kyzan, Champion of the Awakened, Salvarola who is now enhanced by the power of a demon, and then the demon himself, Ultane. Ultane dies, Ascended win.

    Ultane
Demon who appears out of nowhere and forms a pact with Salvarola, he is basically the governing body for the changes in this reality.

    Salvarola
He's got demonic powers now! Pew pew pew! Kills Princess Isabella.

    Magma Lord Kaliban
Kaliban after he's been "baptised in Brimstone and Hellfire", has even more anger issues than usual and is still a big four armed monkey.

    Planebreaker Bastion Raid
Architects plan to achieve dominion over the planes, Ascended race to stop them. Defeating all the minions, they face Inyr'Kta, kill her, kill the Council of Overseers then Planebreaker Abominus.

    Eggtenders
Some Egg Tending Caterpillars. Not much else to understand.

    Proteus
A really big, ugly fish pulled from Myrkur's Depths in the Plane of Water, Inyr'Kta planned on using his essence to fuel Abominus' power, but he was too strong for her Planar Hunters and the Ascended killed them and Proteus.

    Warden Thrax
Jailor for the Bastion's war prisoners, wields a mastery for traps as clearly demonstrated by the jail room and it's various moving platforms.

    Inyr'Kta
The reincarnation of Inyra, one of the Awakened Leaders, is the leader of the Architects till she is slain by the Ascended.

    The Council of Overseers
A trio of architects who appear to have been in charge of the hive before, and are perhaps responsible for the transformation of Inyra.

    Planebreaker Abominus
An enormous robot constructed by the Architects to launch a massive assault on the Planes.

    Infinity Gate Raid
Ascended arrive at the Infinity Gate and kill some Architects, Brothers of Life and Death and Volan. They then enter the Planebreaker Bastion.

    Breaker X-1
Like Kaaz'Ari and Kaaz'Ra, another enormous robot constructed by the Architects, designed specifically for the defense of the Infinity Gate.

Mordan and Viktus, The Brothers of Life and Death
Lore doesn't give many clues about these guys, but it seems "she" could be Inyr'Kta who for a short time looked after them and then they went off to their respective planes which they gained a mastery over (Based on how long it's been since Queen's Gambit, I'd suspect the flow of time in the other Planes is pretty different, or non-existent entirely.), they also have a serious case of sibling rivalry.

    Volan
The Last of the Karthans, big, bad, lizards who once roamed Ashora. Volan was given mechanical armaments to use him to wipe out the other Karthans centuries ago by the Eth. Presumably he escaped his prison in Ashora after the Architects freed him, or something like that.

    Bindings of Blood Raids
Ascended go back in time to stop the Goddess of Fate and her buddy Tyshe from changing history to make it so the Blood Storm are never sealed away (Because this supposedly leads to more dangerous threats the Telarans apparently can't handle, possibly referring to Martrodraum). Ascended defeat them and seal the dragons away once more.

    Fate
Appears during the Binding of Greenscale, is responsible for the attempts at altering history, is slain by the Ascended before Greenscale is imprisoned.

    Tyshe
Appears during the Binding of Akylios, assists Fate and acts as her voice, she is slain before Akylios is imprisoned.

    Mount Sharax Raid
Sharax are planning to unleash Martrodraum upon the world, and they do succeed, but Martrodraum is defeated, along with the Sharax.

    Bulf, Herald of the End
Major Akvan character during the Tarken Glacier storyline, he is now present as the gatekeeper of Mount Sharax (and the Eggtenders of 3.0).

    Jinoscoth
    A Warmaster of the Sharax.

    Izkinra
The Dragon of Ice who Akylios hurled into the cosmos near the beginning of the story, he returns thanks to the summoning of the Sharax Warmasters, and is given a new body.

    Yrlwalach
Not much is known about this one, but it's apparently the full-sized version of the weird thing that sticks out of a Water Rift trying to bite the ground about 50m below it.

    Threngar
Leader of the Sharax, has hopes that Martrodraum will devour the world as vengeance for whatever the Gods did.

    Martrodraum
Devourer of Worlds, Ultimate Nightmare. The Sharax attempt to summon him and inevitably succeed, but he is defeated at the hands of the Ascended.

Hope that gave some more enlightenment on Mathosia zones, as well as all the raids. A few Raids were left out because they were mentioned on my Main Story thread, except PBB/IG because they had some noteable characters involved, which is also the answer to why some of the Raids have every single boss/character listed and some just don't. It's in order of lore value. Thanks for reading, will make sure to make more for Brevane/Dusken/Plane of Water in more detail, as well as some more noteable characters throughout the storyline.

Primeval Feast is an alternate reality where telara is already "conquered/eaten". Twyl/Hylas released green scale who ate all of us, then the remaining Fae killed and ate him(for all that super powered ascended goodness). Hylas helps us because that wasn't how he actually wanted releasing green scale to go down. Plus shyla was totally an ascended, which means she was also eaten.

I didn't put that!? I really didn't!? Ew, probably didn't pay attention then. Didn't bother adding Shyla in because I was probably gonna mention her for the Guardian story, which you probably noticed I didn't bother adding, because I need to do some research on that :P

Edited Primeval Feast, Shimmersand and Ember Isle a little. Thank You Caldern!
